Medical records & test results

You can conveniently access your medical records, test results and prescriptions within NorthShoreConnect. This includes your Health Summary, medications, immunizations, allergies, test results, recent hospital visits and more.

Your Health Summary contains a snapshot of the information that is stored within your electronic medical record within NorthShore University HealthSystem, including current health issues, medications, immunizations, allergies, test results, recent hospital visits and past medical history.

  1. Log in to your NorthShoreConnect account.
  2. Open the menu and expand the My Record section.
  3. Choose the desired topic.

  2. Select Test Results from the menu.

Check number of refills remaining and expiration date

Before requesting a prescription renewal within NorthShoreCConnect, please read your medication bottle to see the number of refills remaining and the expiration date.

  • If you have refills remaining and the prescription is within its expiration date, please contact your pharmacy directly to fill your prescription.
  • If your prescription bottle shows 0 refills remaining or if the prescription is past its expiration date, please follow the steps below to renew your prescription.

How to renew a prescription in NorthShoreConnect

  1. Log in to your NorthShoreConnect account.
  2. Select Medication Renewal.
  3. Choose the medication(s) that you need renewed.
  4. Select Request Renewal, then select Submit Request.
